Immunization Requirements

To keep our community safe and healthy, Sea Island Sprouts requires all children to have a valid South Carolina Certificate of Immunization (DPH Form 4024) on file prior to enrollment.

Military families may submit a DHEC Form 4375, completed by a military healthcare provider and stapled to DD Form 2766 or PHS Form 630, in place of the standard immunization form.

In limited cases—such as recent relocation or difficulty obtaining records—we may issue a 30-day Special Exemption using the official South Carolina form. This short-term exemption is designed to allow time to retrieve immunization documentation. After 30 days, a valid Certificate of Immunization must be submitted to continue attending.
